Many people wonder whether they need to avoid certain foods after receiving a rabies vaccine. Generally, after the injection, doctors advise patients to stick to a light diet and avoid spicy foods, alcohol, and strong tea—and in some cases, even shrimp, crab, beef, and lamb. Is there any scientific basis for these recommendations?
National Immunization Program Vaccines
(1) Vaccines administered
These include the hepatitis B vaccine, BCG vaccine, inactivated polio vaccine, DTP vaccine, DT vaccine, measles-rubella vaccine, MMR vaccine, hepatitis A vaccine, Japanese encephalitis vaccine, and meningococcal vaccine.
Neither the vaccine package insert nor the relevant technical guidelines specify any dietary restrictions following vaccination. Therefore, you may resume your normal diet after vaccination; there is no need to avoid certain foods.
(2) Oral vaccines
Bivalent live attenuated polio vaccine
Instructions: This product is a live vaccine. If necessary, administer it with lukewarm water (below 37°C); never use hot water.
The “Frequently Asked Questions on the Prevention and Control of Polio” (China CDC website) states: “Because the oral polio vaccine is heat-sensitive, it should be taken with lukewarm water at a temperature below 37°C. Hot water or hot food can inactivate the live virus in the vaccine, thereby reducing its immunogenicity. Therefore, children should wait 30 minutes after taking the vaccine before consuming milk.”
The 2-valent polio vaccine is similar to the oral polio vaccine in that it is also a live attenuated vaccine. Therefore, it is recommended that you avoid breastfeeding or drinking hot water for half an hour after vaccination. After half an hour, you may resume your normal diet without any dietary restrictions.
Category II vaccines
(1) Vaccines administered
Including the pentavalent vaccine, 13-valent pneumococcal vaccine, 23-valent pneumococcal vaccine, hand, foot, and mouth disease vaccine, varicella vaccine, Haemophilus influenzae type b (Hib) vaccine, meningococcal A+C conjugate vaccine, influenza vaccine, hepatitis B vaccine (for adults), and others
Neither the vaccine package insert nor the relevant technical guidelines specify any dietary restrictions following vaccination. Therefore, you may resume your normal diet after vaccination; there is no need to avoid certain foods.
(2) Oral vaccines
Oral live rotavirus vaccine
Instructions: Do not take with hot water, as this may affect the immune-boosting effects.
Since the oral live rotavirus vaccine is similar to the bivalent polio vaccine (both are administered orally and are live vaccines), it is recommended that infants not be breastfed or given hot water for half an hour after vaccination. After half an hour, they may resume their normal diet without any dietary restrictions.
Rabies vaccine
The rabies vaccine package insert states: “Avoid alcohol, strong tea, and other stimulants, as well as strenuous exercise.”
1. The instructions for the rabies vaccine are taken from the *Pharmacopoeia of the People’s Republic of China*. Prior to 1980 in China, rabies vaccines were produced using animal brain tissue. They required multiple doses and were administered in large quantities. Additionally, since the vaccines at that time were not purified and contained high levels of impurities, they caused significant side effects. Activities such as drinking alcohol and engaging in strenuous exercise could exacerbate these side effects. Therefore, the vaccine instructions at that time explicitly stated: “Do not consume alcohol, strong tea, or spicy foods; avoid exposure to cold, catching a cold, or engaging in strenuous exercise or physical labor.” With technological advancements, modern rabies vaccines are produced via cell culture and undergo concentration and purification, resulting in a significantly reduced incidence of adverse reactions. However, the 2015 edition of the Pharmacopoeia and the instructions from some manufacturers still retain the previous content. Yet, there is no experimental evidence to suggest that the current rabies vaccines should still include the aforementioned dietary and exercise restrictions.
2. The package inserts for rabies vaccines manufactured overseas do not list these contraindications. Furthermore, WHO documents on rabies have never mentioned any dietary or exercise restrictions, and a review of existing domestic and international literature reveals no scientific basis for the aforementioned dietary or exercise restrictions.
Therefore, dietary and exercise restrictions following a rabies vaccination are not strictly necessary. While you may choose to follow these guidelines, there is no need to be overly concerned about them. If you drink alcohol or eat spicy hot pot after receiving the vaccine, there is no need to worry—these activities will not cause the vaccine to fail. Once vaccinated, you can simply resume your normal lifestyle and diet.
In summary, after receiving an injection, you may resume your normal diet without any dietary restrictions. After receiving two oral vaccines (the bivalent polio vaccine and the rotavirus vaccine), do not breastfeed or give the child hot water for half an hour. After half an hour, the child may resume a normal diet without any dietary restrictions.
